GRANT REQUEST INSTRUCTIONS
Crossroads Community Services, Inc. will consider requests 3 times each year: June, October and February. Your request needs to arrive by the first day of the month to be considered. Organizations will be notified of the decision in writing usually within 2 to 4 weeks.
Please complete these questions:
Name of the 501(c) (3) organization applying for the grant? Please attach a copy of your IRS tax determination letter. If your organization has measurements of past outcomes or successes you believe should be considered, please include.
Describe the program or event to be funded, and how you will measure outcomes of the funding.
Indicate the grant amount requested, and the time frame needed for funding.
Provide contact person, and their contact information for any questions related to this grant request, including mailing address, phone number and email address.
DELIVERY INSTRUCTIONS
The grant request letter must be signed by the contact person and the president of the 501(c)(3). Grant request should be mailed to:
